I've lived with family members as an adult, had other family members live with me at times as well and never had this happen. The only time I've had a second name on my insurance was my very first car and that was because one of my parents co-signed my first loan when I was a teenager. Even now that I'm married my spouse and I each have our own policies on our own vehicles. Not saying it isn't plausible, but it would be a first for me hearing that and I've lived and had insurance policies in Illinois, Texas, and Missouri, all times that I lived with other people. Some roommates, some family, never added any to my auto policies.

Apparently it isn't law, but you will get dropped if insurance finds out all licensed drivers in the home are not covered (by some form of car insurance). I think it is pretty safe to assume OP's cousin did not have their own insurance.
