If (as in my 24 Hybrid) the sound you hear when slowing is louder than the sound when you're starting to move in electric mode, then the slowing sound is in fact the regenerative braking, not a safety sound. I honestly don't hear the forward safety sound from inside the truck.

I can't speak for the east, but in the west, when a pedestrian is waiting at any crosswalk (including the unmarked ones at every corner), it is a moving violation for a motor vehicle to fail to yield so that the pedestrian may cross.
Haven't you noticed? Newer big trucks, delivery vehicles, and construction vehicles have added "white noise" makers instead of beepers (on the loud side of course).
I promised my wife that if I ever go full electric, I will find a way to change the noises to that of a 5.9l Cummins!
It also implies that the driver saw them and kept going, and also that the person can hear, which the driver probably doesn't know. We can't know the specific situation, but it also seems like the driver could see the pedestrian and keeps going, which is at least stupid and rude. Ya, go ahead and hit a pedestrian in a parking lot and see how that works out for you. Where I live pedestrians have pretty good rights in cross walks and while on roads without sidewalks. There are even laws covering the distance a vehicle should keep from them. I personally tend to give drivers plenty of leeway while they're being cautious etc, but they also get good audible reminders if they disregard and disrespect my presence or the law.

Boy that speaker is really hard to get to.
Medium. I could have done it myself but I had a bad back at the time so I just paid my mechanic to do it for me. It does require the removal of the driver side tire to access the speaker
