There's probably no difference at all. Eco mode does more regen during low throttle. On the highway with steady load there no regen.
Unless there's a hilly area with varying loading. I got very good mpg's in the Ozarks with all the constant ups and downs.

I got the cigarette lighter socket dvm and, while probably not as good as testing the battery directly it's still good feedback. Mine has been rock steady on my hybrid for 2.5 years. If it starts deviating I'll know something is up and do more checks.

The chances of a worst case scenario are very small. And that would probably be the brakes be applied and staying that way because of stuck pistons. You'd have some advanced notice probably with pedal feel. A flush would be good but I'll probably wait myself till 100k miles.
There's plenty of jacked up pickups around that are just as difficult. Any cab over or forward cab would be like that. This one might be better lowered.
