Wheel comes off some pickup. What causes that?

I saw this on the road today. The back wheel came off a pickup. What does that?

Loose lug nuts?

Happened to my wife. Picked up my truck from service and lost a tire on the way home. They only hand tightened the lugs on that wheel. They were very apologetic.

Or ignoring that grinding noise and skunky differential oil smell coming from the axle bearing too long.

Maverick lug torque is 150 foot pounds. Probably the same on the bigger trucks. Most vehicles are around 75 to 80 foot pounds
Somebody didn't put enough muscle into it. ALWAYS RTFM and use a torque wrench.
