I had the same problem. Ford "fixed" the issue under warranty. They installed the same problem parts expecting the issue to be resolved. I ended up buying new nylon nuts and using clear silicon sealer to affix them in place. Problem solved.
That's the same item I installed about 8 months ago. It won't throw any codes, simply mimics pushing the mode button. I used 2 credit cards to remove the plastic cover. It popped right off without marring the finish.

I sent the pictures that Mabett requested and received within 4 days the headrest hooks, a cup holder and a fan. They really seem sincere about getting the vent deflectors manufactured correctly.
My 23 XL has done this randomly since new. Only locking the driver's door, regardless of the location of the key fob, or weather. Dealer suggested changing the unlock sequence from default to all unlock with one push of the fob which made no difference.
