Step 6 in the instructions. I've located the hole, but the picture makes it hard to figure out which direction the screw with plate should fit into the drilled plate. And from there, it's hard to pull the screw with plate through the drilled plate
Didn't want to leave you completely hanging.
I didn't do an under the hood breakout box, I ran all the wiring to behind the rear seats since I have a hybrid and the 12v battery is back there. Plus my siren box can't get wet. I build a small board that went on top of the storage cubby and made a...
The wheel well one always seems odd to me, but if you've got the clearance, you should be good. Everything else looks like decent placement if you're comfortable drilling holes into those trim pieces and running wires. Just make sure to set all your lights to the same pattern and use sync wires...
I'll work on uploading mine, but I used a combination of Feniex, FedSig, and Hella products. Hella has a plug and play replacement bulb for the rear backup lights that was very handy. I wired everything myself. I didn't want to do too much drilling/modification of the body to attach lights in...

We have a similar concept application on our fire engine doors. I'm not smart enough to know how to limit it to just the one door that was opened, so that your passenger door isn't blinking in the cab when only your driver door is open. Your mileage and knowledge may vary
Depends on how fancy you want to get. You could potentially add something like the Feniex T3, set the flash pattern to a single flash, and wire it off of the dome lights to come on when the door is opened
